Vacaville American Little League Registration Checklist
To have your player fully registered and eligible to participate, all required forms and documents must be submitted both online and to the Player Agent at [email protected].
Required Documents
- Player’s Birth Certificate
- A copy is required for all players.
- If your player was registered last season and a copy was already submitted, you do not need to resubmit.
- Medical Release Form
- Completed and signed every season.
- Proof of Residency OR School Waiver Form
- Proof of Residency Documents: Three specific documents dated between 2/1/25 and 2/1/26. Contact the Player Agent for the accepted list.
- School Waiver Form: Accepted if your player attends one of the following schools: Alamo, Bethany Lutheran, Browns Valley, Hemlock, ACE Program, Jepson, Kairos, Markham, SPICE Program, or Orchard.
- Media Release - Signed
- Parent Code of Conduct Form – Signed.
- Player Code of Conduct Form – Signed.
- Volunteer Code of Conduct Form – Signed
- Required for anyone with direct interaction with players: managers, coaches, assistant coaches, scorekeepers, Board Members, Team Parents, etc.
- Background Check: Must be completed every season. Requires Social Security number and a Driver’s License/ID. You are ineligible to volunteer without a completed background check.
Important Notes:
- To be eligible for end-of-season Tournament Play or All-Stars, your player’s original birth certificate must be available and submitted to the Player Agent if selected.
- Players League Age 8 and above may be selected for All-Stars.
- Players are ineligible for All-Stars if the original birth certificate is not provided.
